(1) Baby Card applicants
- When applying for birth registration, qualified applicants (such as parents, grandparents, heads of household, conjugal partners, or adopters) can apply for the Baby Card at the same time.
- Father or mother.
- Guardian.
(2) Documents for applying for the Baby Card.
- Household register and the citizen identity card and personal chop (signature) of applicants.
- Color photos of the newborn/child or family photos, the electronic file of photos with memorial value (please provide a high-resolution version of photos in the electronic file or sharp and clean photos).
