What are the requirements for applying for draftee physical status change?

After the conscription examination and before conscription, a draftee has doubts about the physical status determination or suffers from newly diagnosed illness should submit a certificate of diagnosis issued by a medical institution that can change the determination of physical status to apply for a re-examination to the township (town/city/district) office in accordance with any of the following methods for the township (town/city/district) office to refer the case to the municipal, county, or city government to review the case. When rejecting the request for re-examination, the municipal, county, or city government should specify the reason for rejection and notify the draftee.
  1. Re-examination with government fund: Complete an application form. If the application is approved, the draftee should immediately contact the re-examination hospital to take the re-examination. The conscription examination committee should determine the draftee’s physical status with reference to the re-examination results.
  2. Re-examination with personal fund: Complete an application form. After being approved for re-examination, the draftee should take the re-examination from the selected re-examination hospital, which should directly deliver the re-examination certificate to the conscription examination committee to determine the draftee’s physical status with reference to the re-examination results.

If the physical status determined with reference to (1) and (2) is the same as the original determination, the draft should not apply for a re-examination for the same reason.
